Offers are being requested and a separate written solicitation will not be issued. Solicitation number 36C25726Q0755 is issued as a request for quotation (RFQ) for Mobile PET/CT Lease. IAW with 52.212-2 Evaluation Commercial Products and Commercial Services: Evaluation of offers will be conducted by comparative evaluation of offers. Technical capability will be assessed with the documentation provided by the offeror (see addendum to FAR 52.212-1, Instructions to Offerors). Past performance will be assessed by conducting reviews and/or interviews with the past performance references provided by the offeror and by reviewing the offeror s past performance records found in government past performance record systems. Price will be assessed as for a best value for the government that is determined fair and reasonable. The government intends to award a contract from this solicitation that represents the best value and most advantageous to the government, price and other figures considered. Factor 1: Technical: Mobile PET/CT Mobile Unit and PET/CT Scanner IAW the SOW, in addition to those required by Federal, State, and Local Government. Certified, licensed, or otherwise officially recognized by the local, state, or regional government or public entity where the Mobile PET/CT Lease services are operated or by which it is governed. Capability and capacity to meet the services required by SOW as demonstrated by Section A and Section B of the SOW. Factor 2: Past Performance. Recent, Relevant, and Successful Past Performance in the Performance of Mobile PET/CT Lease Services commensurate with the magnitude of the services required per the SOW. Factor 3: Price. A sum of all LINs for the base year plus 4 option years using. As part of price evaluation, the government is evaluating the option to extend services, under FAR Clause 52.217-8, by dividing the price of the last Ordering Period in half to determine the price for the 6-month extension. Pro-rated rate in the event that the mobile PET/CT in any event that the system is unoperated beyond 48 hours. Past Performance: Offeror shall provide at least two (2) references for similar contracts in the public or private sector. These references shall be recent (last five years) and relevant to the required services as stated in the SOW. The Offeror shall provide no more than five (5) references. The award will be made as the response most advantageous to the Government. Responses should contain your best terms and conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoters shall list exception(s) and rationale for the exception(s), if any. Price: Submit price on B.3 PRICE SCHEDULE information on a separate page/document. (Quotes shall be valid for a minimum of 60 calendar days). The Government will evaluate the price by summing all priced Line Items (LINs). As part of price evaluation, the government is evaluating the option to extend services, IAW FAR Clause 52.217-8, by dividing the price of the 1-year LIN in half to determine the price for the 6-month extension. The Total Evaluated Price will be that sum. The Offeror will provide a prorated rate in any event the system is unoperated beyond 48 hours. The Government will evaluate the price by summing all priced Line Items (LINs). As part of price evaluation, the government is evaluating the option to extend services, IAW FAR Clause 52.217-8, by dividing the price of the 1-year LIN in half to determine the price for the 6-month extension. The Total Evaluated Price will be that sum. Offerors shall submit all questions no later than Tuesday, August 4th, 2026 by 10:00 AM CDT via email to Contract Specialist (Gillian M.
