Statement of work, as posted

Veterans Health Administration, Network Contracting Office 5, is issuing this request for information/sources sought notice to identify companies that have the resources and capabilities to provide a door for the NW Entrance of the Clarksburg VAMC. The Louis A Johnson VAMC has the need to sole source the Procurement of the NW Entrance Door that serves patients and staff. The temporary structure is being removed, the existing door will need to be moved and will not fit the space allotted following removal. The Louis A Johnson VAMC is looking to procure by way of Sole Source Justification, a contractor to provide a new door, provide all parts and labor to deliver and install the door at the Northwest Entrance that serves Building 1. Description of work: The contractor shall provide all parts to install the new door, a new single left-hand sliding door with transcom, electric locks, and panic devices to meet NFPA 101. Door will be constructed of clear glass above and below the transcom, and aluminum frame. All labor for the service technicians to make the necessary installation and programming, and travel to and from the Louis A. Johnson VAMC. The Louis A Johnson VAMC shall provide 1 Locksmith for the purpose of oversight of the repairs, 1 Electrician for the purpose of shutting off power to the doors, to include the Lock Out/Tag Out materials for the breaker, and infection Control Barriers if required by Infection Prevention Staff. Technical Qualifications: Contractor must be qualified to operate within an active healthcare facility without compromising air quality or spreading biological contaminants. Must have ICRA (Infection Control Risk Assessment) Training, OSHA 30-Hour Construction Certification, and Understanding of NFPA 101 (Life Safety Code). Interested & Capable Responses: NCO5 is seeking responses from companies that are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide these services. Confirmation that your organization has the capability & expertise to meet the performance requirements while complying with applicable limitations in subcontracting by responding to the questions below with your response: a.
