REQUEST FOR QUOTATION (RFQ) – BIOHAZARDOUS WASTE MANAGEMENT SERVICES The Indian Health Service (IHS), Albuquerque Area Office (AAO), is soliciting quotations for comprehensive Biohazardous and Regulated Medical Waste Management Services for the Santa Fe Service Unit. Services will be provided at the Santa Fe Indian Health Center, San Felipe Health Clinic, Santa Clara Health Clinic, and Cochiti Health Clinic. The requirement includes, but is not limited to, collection and removal of regulated medical waste, sharps collection and disposal, transportation, treatment and final disposal, container replacement and maintenance, manifest and tracking documentation, annual staff training, and as-needed/emergency response services. Complete requirements are provided in the attached Statement of Work (SOW). This acquisition is set aside for eligible Indian Small Business Economic Enterprises (ISBEEs) under the Buy Indian Act and applicable IHS acquisition requirements. NAICS: 562112 – Hazardous Waste Collection PSC: S222 – Housekeeping – Waste Treatment/Storage Contract Type: Firm-Fixed-Price (FFP) Period of Performance: One (1) 12-month Base Period and four (4) 12-month Option Periods The Government intends to award one contract to the responsible quoter whose quotation represents the best value to the Government upon considering Technical Capability, Past Performance, and Price. Technical Capability and Past Performance, when combined, are more important than Price. Interested quoters shall review the complete RFQ and all attachments for quotation preparation requirements, evaluation criteria, applicable wage determinations, and submission instructions. Questions and quotation submissions shall be provided in accordance with Sections L and M of the attached solicitation.
