AMENDMENT 2 Description of Changes: The purpose of this amendment is to: Provide a Revised Excel Attachment: Correct a technical formula cell discrepancy on the Base tab (Cell K5 Burden Rate) of Attachment 3. Mandate Revised Template: All Quoters are required to use the file titled "Attachment 3 Updated - Amendment 2" for their proposal submissions. Submissions using older versions of this attachment will be deemed non-compliant and will not be evaluated. Extend Submission Due Date: The proposal submission deadline has been extended to Tuesday, August 25, 2026, at 2:00 PM PDT. Acknowledgment Requirements: Offerors must acknowledge receipt of both Amendment 1 and Amendment 2 within their final quotation package. AMENDMENT 1: Adding Compensation Plan/Pricing Sheet for proposal submission. Erroneously omitted from original RFQ Updated Instructions: FACTOR 1: Compensation Plan: The Quoter shall submit a Compensation Plan, using Attachment 3. The compensation plan shall include enough information on to demonstrate how they will recruit, compensate, and retain the required Food Service Worker to perform the required services in accordance with Attachment 1 (Performance Work Statement). At a minimum, the plan must clearly identify: 1.??The unburdened hourly wage proposed for the Food Service Worker. 2.??The specific Fringe Benefits offered (e.g., Health & Welfare rate, Paid Time Off, Sick Leave compliance with EO 13706). 3.??The Quoter's methodology for ensuring continuity of service (e.g., how the Quoter will handle unexpected absences, sick leave, or employee turnover to ensure the MOMMC dining facility remains staffed). Offerors are cautioned against simply restating PWS requirements in their proposal. Statements such as "the offeror understands" or "the offeror will comply with the Service Contract Act" do not reflect a clear understanding of the retention requirements and will likely result in a diminished evaluation rating. FACTOR 3: Price: The Quoter shall use Attachment 3 to submit a firm-fixed-price for all contract line item numbers (CLINs) identified. Attachment 3 shall be completed and signed, with acknowledgment of all amendments to the solicitation. No attempt to reformat or remove cells is authorized. Submission of pricing data shall be complete and accurate. Incomplete pricing may result in a Quoter’s quotation being deemed ineligible for award. The Defense Health Agency Contracting Activity (DHACA) Healthcare Contracting Division-West (HCD-W) requires non-personal services to provide one (1) Full-Time Equivalent (FTE) Food Service Worker (SCA Occupation Code 07130) to support the Nutritional Medicine Department at the Mike O’Callaghan Military Medical Center (MOMMC), located at Nellis Air Force Base, Nevada. The contractor shall provide all personnel, management, and uniforms necessary to perform food preparation, sanitation, and cafeteria support services in accordance with Attachment 1 (Performance Work Statement - PWS).
